pCare Ambulatory is a mobile patient engagement solution designed to:

How Does pCare Ambulatory Work?

To demonstrate how pCare Ambulatory works we will follow an example patient named Bob:

Bob is diagnosed with coronary artery disease and together with his doctor, Dr. Jones – they decide that Coronary Artery Bypass Graft (CABG) surgery is the best course of treatment. To prepare for the surgery and recovery, Bob agrees to enroll in the pCare Ambulatory CABG Careflow. As a result Bob receives a personalized invitation and validates his identity with his birth-date to enroll in the Careflow with no applications required to download!

Upon login, Bob can see important information about his surgery – such as the date and care team members. The hospital-branded digital engagement includes articles, videos, interactive checklists, and patient feedback designed to match a variety of learning preferences and sequences to guide Bob through his care journey with turn-by-turn navigation answering the question of ‘what is next?’

Before Surgery

Before surgery, the Careflow prompts Bob to complete a variety of self-assessments, symptom checkers, and check-ins which trigger notifications to the Care Team when follow-up is required.

Bob can reach out to Dr. Jones from within the Careflow with any questions, enabling convenient, streamlined asynchronous communication for non-critical issues.

All Interactions with the Careflow are tracked and accessible through the Caregiver portal. As the surgery date approaches, Cindy, a nurse coordinator on Dr. Jone’s team, is notified that Bob is falling behind on his Careflow and reaches out to see if she can help get him back on track.

The day before surgery, Bob receives a prompt to complete his pre-surgery checklist to ensure he arrives prepared for his procedure.

Admission and Post-Operative Care

At admission Bob’s Careflow becomes available on the pCare TV system. Bob and his family can continue to prepare for his procedure and recovery.

Upon discharge from Bob’s surgery, his Careflow follows him home to support him through educational surgical site care and rehabilitation exercises, prompts for pain assessments and other resources to guide him on his recovery journey.

Three Months Post-Surgery

Bob wraps up his Careflow when he submits his 90-day post-surgical PRO. He is satisfied with the outcomes and confident in his ability to stay healthy that he recommends Dr. Jones and Market Street Health to several of his friends. Dr. Jones is pleased with another successful surgery with no complications and for Market Street Health, the promise of a long-term relationship with Bob and his family.
