Simply put, remote patient monitoring is a telehealth service that connects patients and physicians through Internet-enabled devices such as blood pressure monitors, glucometers and smart scales and a digital health platform. These patient monitoring systems transmit and analyze data about the patient’s vital signs, body weight and other relevant condition information to doctors and help them make more-informed decisions.

RPM in healthcare works by giving qualified patients a connected-health device that they use to take readings at a regular interval as determined by the specific program. For instance, for a type II diabetes program a patient may be given a FreeStyle Libre CGM device, a digital weight scale and a blood pressure cuff. Through the use of the Mozzaz Platform doctors and other caregivers can access key insights at-a-glance in addition to detailed logs to ascertain trends, observe the impact of lifestyle choices and track the overall effectiveness of treatments. By empowering care teams with a holistic view of patients outside of the traditional care setting you can ensure the best care outcomes.

We offer different pricing options depending on the specific type of program and the needs from our variety of tools and features. Based on your patient volumes, we can provide a clear, upfront estimate of costs.

As directed by Medicare, clinical staff may manage and support RPM services while under the general supervision of the billing provider. Specially, for CPT codes 99453, 99454, 99457 and 99458, the billing provider is required to be a physician or another qualified healthcare professional (QHCP).

Absolutely! A care provider may bill both the RPM CPT code 99457 and CCM CPT code 99490.
CMS recognizes the synergies between analyzing RPM data and services and other chronic care services. That being said, the time spent towards each of these may not apply to the other.
In simple terms no double counting, billing both 99457 and 99490 require a care provider to deliver a minimum of 40 minutes of services: 20 minutes towards RPM, and 20 minutes towards CCM.

Yes, documented patient consent is required to receive care through RPM. Mozzaz offers digital consent options and patient-facing educational material to help.
